Wesley Family Services' Mental Health Program: Support and Crisis Intervention
Wesley Family Services Family Based Mental Health Services (FBMH) caters to complex issues, offering family therapy, crisis intervention, prevention services and service coordination for individuals under the age of 21 and their family.
The approach is designed specifically for those that need a more intensive option.
How Does Family Based Mental Health Services Work?
With the eventual goal of reducing the need for higher level care, the FBMH team begins by meeting with the client and family multiple times a week to perform a comprehensive assessment. The team will focus on evaluating the familyís unique strengths, challenges and needs, in order to offer the interventions best tailored to helping them reach their goals.
Because FBMH is uniquely collaborative, the family, team and providers will work together to develop personalized treatment goals and objectives, which will then be implemented into an official family treatment plan. This plan will be updated and reviewed on a monthly basis with the familyís input. The team will also work to create a crisis/safety plan that will be revised with new skills and ideas the family utilizes while participating in the program.
After these plans are developed, the team will base treatment sessions on them moving forward. A unique facet of FBMH is the integration of different areas of the childís life, so sessions will include the whole family and different subsets within the family.
The team will also include treatment outside the home as needed, including at medication checks, school meetings, community meetings, etc. After each session, homework is developed with family members to build on skills that have been practiced in-session.
After Family Based Mental Health Services
The duration of FBMH is different for every family, since the model takes an adaptive approach.
However, families have the most long-term success when services and support are continued after FBMH. Together, the family and care team will find and discuss the best aftercare options, and 8-12 weeks prior to discharge from Family Based, referrals will be made to outside support options to allow for the most seamless transition possible.
Eligibility:
The program treats individuals under the age of 21. Applicants must have a current behavior health diagnosis.
The person should be determined to be at risk for a higher level of care, such as inpatient psychiatric care, residential care, or foster care, and may be in a situation where other lower levels of care have not been effectively (such as outpatient services, school-based services, or Behavior Health Rehabilitation Services.)
Recommendations for Family Based Mental Health Services should be made and documented by a licensed physician or psychologist, but families can self-refer and seek a referral retroactively.
